Fall 2024 Learning Session Highlights Health Equity & Population Management

We are grateful for convening another productive Fall 2024 Learning Session! We gathered as a community to understand ways to use Phlox registry population management features to improve care, learn about opportunities to integrate health equity into PR-COIN quality improvement (QI) activities, promote health equity through enhanced data capture and stratification of QM reporting, monitoring impact of interventions on health disparities, and more. 

Highlights from this Learning Session on putting data into action to improve quality of care include: 

  • We heard a wonderful keynote address from Alicia Tieder, LICSW, MBA (Seattle Children’s) Associate Vice President & Chief Health Equity Officer. Ms. Tieder laid out a roadmap to advancing health equity at a system-level, including taking steps to collect important patient demographic data, using this data to identify disparities and inequities in care outcomes, and utilizing QI to create interventions and track progress, and be inclusive of all patients in eliminating disparities. 

  • Parent Advocate Jen Marshall (Children’s Hospital of Philadelphia) shared her family’s story and challenged our network and beyond to care for the whole child, including mental health, when caring for JIA. Patient Advocate Kat Wilder (Boston Children’s) shared her inspiring story of overcoming obstacles in her care journey to achieve her goals. 

  • Mark Clement, MD, PhD, CPI, FAAP (Children’s Mercy – Kansas City) shared a real-life example of population management within the endocrinology context. Utilizing predictive modeling and remote glucose monitoring, Dr. Clement and his team have made huge technological strides to advance diabetes care and have also achieved the first national value-based care subspecialty contract in pediatrics. 

  • Kristen Hayward, MD (Seattle Children’s) and Mary Toth, MD (Nemours – Orlando) share the telemedicine improvement change package, with all materials developed through a grant from Pfizer.
